Designating a Beneficiary

You can designate a beneficiary for any loss of life benefits. To designate a beneficiary, contact us to request the necessary form to complete and return. 

If no beneficiary is designated in writing, benefits will be paid in the following order:

(1) Your lawful spouse if not legally separated or divorced; 
(2) In equal shares to your children; 
(3) In equal shares to your parents; or 
(4) your estate.
